Pan Card — Incorrect Name on PAN Card

I have Applied for Pan Card way Back in 2008,Although i receive it on time,but my Date of Birth was Incorrect,so i submit it back and filled a correct form for getting my Date of Birth corrected.It took not less than six months for them to correct it.after getting my D.O.B correction another Mistake surfaced -First and second Name were clubbed together,At that time i don't noticed it and due to delay in getting my D.O.B,I have not so much time to follow it,tell me,how could it get it corrected after one and half years have passed since I Received it

Asper your Jammu office that we they have despacted the Pan Card through there runer but till date no one has conceted at our communication adress given in Pan Card application nor any Phone call has been received from your Jammu office.When we concet them after geting Pan application status they said at they have not located the communcation adress nor respond given Mobil No. which is a false statement
It is requested to enquir about the matter and Pan Card may delivered at communcation adress as early as possible so that I will not suffer anymore.

Have been trying to catch up with you for his PAN card but in vain. Do not understand where to get through for the same as time for submitting the details to Infosis companhy, Mysore where he is doing his IT training after completing B.Tech is at verge of completing and I need to send these details to him. I had applied for PAN CARD.My PAN CARD made and dispached to the address which was given in PAN CARD Application form.I had contacted to PAN CARD Service center they said that your PAN CARD has dispached to the given address by courier(FIRST FLIGHT) and received by a person name is RAHUL but I could not received my PAN CARD so far.So i request you to kindly do the needful as i facing lots of problem without PAN CARD.
