Panind — issuance of duplicate card - have not received application form

I have applied for issuance of duplicate pan card and have paid the required fee as per documents attached herewith. I could not download the application form after the amount got deducted and have been trying to contact their customer care but none of the numbers are reachable. Also, they had mentioned that they send the application via email, which i haven't received yet.

The payment details and order number are attached herewith.
