Dear Sir,
I wish you to in-form a complaints under the invoice no :2019-20/IN[protected], vide Patients (RAMJILAL) ID ;[protected], , and Receipt no 2019-20/IP[protected] dt 09/11/2019 ( ( MR -402483 ).
That, I am regularly awaiting since, for phone calls since under treatment vide above-mentioned invoice, MR no, receipt no, which is enclosed as per appointment given me by Dr..AMIT SHARMA, at . Clove Dental, HOSPITAL Patel – Jalandhar, 2nd floor, patel Hospital.Civil Line’s Jalandhar Punjab, India 144001 then on different dates, but at the final stage crown fittings process is still pending on date .
Also, the material fitted in the same tooths of mine for the restoration – nanohybride -Composite is going demaged/ lost, due to sub standard quality materials fitted as results of its now a days the area surrounding the pending crown to be fitted, creating infectious, polishing to the composite materials, etc which is under warranty period is also going expired stand by.
its requested to kindly look into the above said matter at your end and call me for follow up at my cell no .below mentioned in this regard as being frustrated pain’full, treatment through your senior consultants Drs .
Hence, kindly take my pending treatment seriously to avoid any further delay which is damaging the surrounding the tooth s & more complexities is going on increased. /decaying day by day’s to mine tooth s as being RCT, PFM, post tooth using rotary files with endormotor, permanent filling X-ray s etc treatments at your such largest internationally glove care hospital 🏥 Brand. Hospitals response of your representative Doctor s, which is not acceptable and arrange & repairs at the earliest at the your end or otherwise provides me fresh treatment under the warranty period,
