This for your kind information that my brother bhushan kumar was bring to pgi for iq assessment test for echs membership referred by military hospital chandimadir, his registration was done vide cr no.[protected] as on 27.08.2018 and he was attended by dr. Dalton senior resident doctor and iq assessment was done on 27.08.2018 against the payment of iq assessment fees of rs/- 100.00 vide receipt no [protected]/1 dated 27.08.2018. Sir i would like to bring to your kind notice that iq assessment report of my brother is not released till date. We contacted number of times to the dr. Dalton on phone and by visiting to the hospital. Copt of opd slip, payment receipt and referral slip is attached with this letter for your ready reference.

In view of the above i request you to please look in to the matter and help us in getting the iq assessment report of my brother as soon as possible. We shall be very thankful to you.
