I, mr. Harendra nath ray, resident of vill-teghoria, po - gandhrbapur, ps-baduria, dist-north 24 parganas enlisted myself as a member of phenomenal health care services ltd on 4th april, 2005, (Membership no: gma[protected], gma[protected]) providing membership fees of rs 12500, 20000, which was to be refunded on 03.11.2015 as per the rules of the policy.
After 9 years of membership as per policy company provided some cheques of dated 10.10.2016 for refunding the amounts but the cheques were not honoured at time.
