Randstad India — my last working day is not showing on pf claim form

This is shaikh shama ismail. in 15 july 2013 i joind nielsen company as a QC auditor on randstand pay roll on 31 march 2018 i left my job, now i want to withdraw my provident fund but i m having a 1 problem of my last working day date which is not showing on claim form i want my full & complete sattlement.
please guide me what to do know, its my kindly request.
