Randstad India — pls update my date of leaving, in epf

I am amin malim and am an ex employee of randstand india pvt ltd., my date of joining 01/09/2015 and i had resigned on 15/04/2016

My employee code was 483855, my pf no. Was tn/35791/427588, uan no.[protected]

I want to withdrawl my provident fund but not able to apply on..
Online portal because my exit date is no updated.

I request you to update it or suggest me solution.
