Dear sir/maam
With reference my conversation with you regarding my policy issue, i hereby going to mention all the issues regarding my policy.
Old policy no/client id: 51963489 –in the name of deepak shukla with dob ; 11.11.1986
Which should be renewed and i had given a chq to mr. manish - religare lucknow office.
Mr. manish did not renew my old policy and simultaneously issued a new policy[protected] care policy)
Against me which i do not want.
So i hereby request you to cancel the new policy and renew my old policy immediately.
Kindly communicate me after doing all the necessary rectification at your end.
