Ringing Bells — Freedom 251 in loyalty card not delivered

I joined silver card membership in order to get a freedom 251. It was given that it will be delivered by 31st december but still it isn't.
These are my details. They gave me the card and got rs.650/- from me instead of 550/-. But still i didn't get my phone delivered.
