This is to inform you that my mother Smt Raso devi died on 26/08/2020 and five fixed deposit Rs. 9000/- each total value Rs 45000/- had been done to your good office sakti branch under Ro Jhanjarpur.
nomination also has been registered in the name of my father Shree shivjee thakur who is not in the well condition and suffering from leg fracture. Even after all the documentary verification have been done by your regional office and also death claim has been issued sector officer is denying to make payment.
In this connection you are requested to kindly verify the attached death claim documents which have been verified by Regional office Jhanjarpur
