Set Top Box — Fastway set top box company is not giving responses against my complaints

Sir,
I am Basara Ram from Sirsa haryana. I have an set top box of fastway company whose MAC no. is 0880393ED0F9.
It is not working from 57 days properly. I have made many complaints to my cable operator. Now he is not attending my call. I have made many complaints to fastway company also but they did not resolved my problems till now. The concerned cable operator misbehaved with us many times.
The mobile no. of cable operator is [protected].

The problems are as follows:-
1. Set top box is not working regularly many time is remain unserviceable.
2. The concerned cable not giving us any receipt of the payment.
3. The address of our house is wrong in the domain of fastway company.
4. The cable operator do not taken any proof of identity or anything else . It might be because of set top box is old.
5. The accessories of set top box are not provided to us yet.
Please help me in resolving these problems created by the cable operator.
