Subject: soft copy of cect done at sir ganga ram city hospital on 26th may 2016 on atul yagnik, male 53 years, registration no 69376, ip no. / bed no: 16/[protected], period of stay: 26th may 2016 to 31st may 2016
I was admitted at sir ganga ram city hospital in room no 407 from 26th may 2016 to 31st may 2016 for diverticulosis with diverticulitis under care of dr. Anil kumar arora.
Cect was done on 26th may 2016 for which the hard copy was taken by the hospital for submission to tpa.
I requested the cect department at sir ganga ram city hospital to provide a soft copy of the cect photos/ film in cd or pen drive. I was ready to pay for it. But i was told to my great surprise that neither the cd writer is functional nor is any usb port for pen drive functional.
Please arrange to provide the soft copy of the cect photos/ film for future reference failing which i shall be raising the matter further for necessary action. Was this information helpful? |
