The insurer denied claim sighting the below:
It is observed from the medical records submitted during cashless processing that the insured patient has undergone surgery 12 years back. Hence, we requested the insured to furnish the past investigation reports and complete treatment records. We note that you have not furnished the required documents and details. In the absence of the above ocuments/details, we are not able to further process your claim.

I have not gone through any surgery in the past before the one done at Medanata in Jan 2024. I have requested Star Health to provide the details of their investigation however the claim is denied.
How do I submit a report of surgery from 12 years before when I have never been operated prior to Jan 2024.
