I want to correct my name in my EB card, which is wrongly entered as "P.Suresh xavior".

My actual name is "J.T.Suresh Xavier".
My service connection NO.:[protected].
My address: 201E, Jubilee Flats, Sithalapakkam, Chennai- 600 126.

I have enclosed the Tax receipt of my flats.
