Please be informed that after submitting the PUK number you need to enter the pin number as 0000 or 1234. Your device will ask you to confirm the PIN again upon which you will have to re-enter the pin number as 0000 or 1234 to access the service. Kindly note that PUK number must be entered correctly and incorrect number entries should not exceed more than 9 times or else your SIM will be blocked.
