In epf passbook show last contribution 09/2019

When I apply online for pension withdrwal, it reject and given cause are, contribution of 09/2019 are not in count.

For change date of leave they need joint declaration form.
But my address is haldia, and your haldia office are not in hare. So how I sign joint declaration form.
