Uco Bank — account debited but cash not dispensed from atm

I, sanjay sharma, having savings bank account no [protected] with uco bank, southern avenue branch, kolkata – 700029, submitted written complaint to my branch manager on 21st september 2017 regarding my account debited but cash not dispensed from hdfc bank atm at lake avenue branch, kolkata - 700026. But nothing has been done till date. I also tried to complaint at "https://www.ucobank.com/english/usercomplaintform.aspx" but this website is not working properly and asking me to fill details when i already had filled every field properly.

On 11/09/2017 at 6:04 pm my account was debited but cash not dispensed from atm as mentioned above.
