UCO Bank — debit card payment failed

I was trying to add money in my paytm wallet using my uco debit card but the payment failed. I tried it 2-3 times but the payment was failing everytime. It had happened earlier also it is working in atm but is not working for online payments.
Sometime it shows it needs bank approval, but after sometime it shows the payment is failed. This is not leting me do any online transactions.

I was using the net banking but before 1 month showing me the user id is disabled for login. I meet with branch manager about this BM told me give a application.I given the application to manager of my home branch till now my problem same that's why please check it and please do the active my user id. My A/c No. SB [protected]
