Up Board Allahabad — Father name and date of birth change in xth marksheet and certificate

Sir,
I request of the applicant that i have passed the state high school examination board first division (70.33%) on 2004.
On receipt of the markssheet and certificate showed it, my birth certificate and the father name is printed incorrectly.

i contacted with Documents in respect of which my own school (Baldev smarak higher secondry scool kharagpur Kanpur) by the Principal on this subject.
he assured me, that he will go to make it right from the board. I had given them all Documents and remained in constant touch with them.
A few years later, he told me that he can not correct your documents and reurn them.
My further studies in accordance with the correct date of birth and the name of the father continued.

I reported in the DIOS Office in Kanpur in 2004-05.
But no action had not yet taken.

Sir, kindly take appropriate action to correct my date of birth and father name.

Would be great.

My correct date of birth is 12/15/1989 which is marked wrong as 12/15/1987 printed
Father name RAJENDRA PRASAD SACHAN which has been marked wrong as Rajendra Kumar Rajput.

My 10 class information as follows...
Name: Kaushal Singh Sachan
Father: Rajendra Prasad Schaan
Date of Birth: 12/15/1989
Roll Number: 0979752
