Up Board — lost 12th marksheet and certificate

I would like to inform that i vishal mishra s/o ramchander mishra had appear for standard 12th examination conducted by up board for the year 2016 - 17 and have cleared the examination in the same year.
Unfortunately i lost my 12th marksheet and certificate. It is a humble request, please issue a duplicate copy of my marksheet and certificate as soon as possible
I need them for my further education programmes as a proof of my 12th clearance.
