My name is K Veeraju have gone for tests with registration no [protected] attached herewith for your reference.
I have shown the prescription of doctor to notedown the tests to your billing department and accordingly paid the amount.
Later I have given blood samples in fasting and pp blood to your sample collection person Mr. LOKESH.
I went for collection of reports and found that FBS & PP REPORTS were not there due to non payment for these tests.
This was informed to jagadaba branch visakhapatnam Incharge Mr.D.Somesh.
Due to non availability of that fasting reports, my brother K Veeraju [patient] who have to go to specialist today evening was cancelled and tommorrow holiday.Re appointment of Doctor will take place in next week and my pp blood sample was wasted due to your fault which pp blood sample was shown to your incharge.
Please advise the concerned to work systematically else ur reputation will be lost and give me for free fasting and ppbs tests due to my repetitive trip to ur laboratory for waste of time and money for trip to centr.
Please inform to vijaya prasoona, zonal head mam also.
