Wipro — Transfer Request Rejected at EPFO office

I have raised request to transfer my PF amount to my current employer and it got rejected at EPFO office stating that the below reason in the screenshot. I could see my DOJ in Wipro showing that[protected] in EPFO porta. Can you please help us to resolve the issue and transfer smoothly.

Date of Leaving : 30th November 2015

Remark for rejected : Your Claim [ Claim Id - PYBOM[protected] ] has been rejected due to :
1) YOUR DOJ(14.11.2011) ON 4PS, 5PS DOES NOT MATCH WITH UAN PLS VERIFY WITH YOUR ER
2) FATHERs NAME DIFFERS
