Sir, I am Chetan & Slomi Sukhadia, here by request you to Refund our FDR Maturity for the FDR No: 1224585, for Rs. 25, 000/- and FDR No: 1224584, for Rs. 25, 000/-, as due since 13.07.3016.
The Original FDR's has already Submitted on 29.06.2016 for the Refund of the amounts.
This on Medical Grounds, I request you to please Refund our money.
My wife Slomi Chetan sukhadia, was having thyroid, due to which her weight had increased
Due to which her, she was facing lot of problems in walking/ Running / Picking weight & just to climb 2 floors she take time more than 20 minutes, and hence our Doctor Adviced to go for an Bariactric Surgery.
Hence she was admitted in Wochardt Hospital, Mumbai Central, Mumbai - 400 011, on : 21.10.2021 to 24.10.2021.
She was treated under Dr. Raman Goel, of Wochardt Hospital.
I am supporting the Hospital Medical Expenses Bill Occured.
Out of Total Bill we had Mediclaim Policy from which we got Rs.1.50 Lakhs & the Balance amount we had to arrange & Paid Rs.4.41/- lakhs.
The Balance amount was arranged from our family & friends, within last 2 yrs, 50% is still due to pay our Family as on date.
Hence I urge, pls Refund my maturity amounts
Of both FD, as we can pay our liabilities.
