Lost CDMA Handset

Dear Sir, I have lost my RELIANCE CDMA Handset in metro .so, Please register my compalin and provide me step to get new sim of same number i.e [protected]

I have already paid in advance Rs. 2000.00 on 04/06/2014 with receipt no 106525 at Kanpur but my cell service is blocked due to non payment . Why ? I am senior citizen and using reliance from past 15 years and never became defaulter
