Kotak Life Insurance — Complaint for non-receiving of insurance certificate

My self, ganesh chandra sen. I am kotak life insurance policy holder. My policy no is 3058653. Plan name is "kotak premier endowment plan - regular - kpe". My premium amount is rs. 20, 004 and started this plan on august, 2014. Till now i have not received any insurance certificate. Please send my all statement and insurance certificate in my e.
