Accenture India — EPS transfer issue

I am Harrishma M M and i have worked in Accenture from Oct 28th, 2021 to April 1 st, 2024.After that I have joined in DH Healthcare company on April 15th 2025 .At that time when I am filling the form 11 1)whether earlier a member of EPS, 1995 by mistake I have given as NO, so my pension amount didn't get transferred to DH health care company .So I kindly request you to take action on this .I have attached my working history in accenture and DH health care for your reference
