Accenture India — updation of employee name in pf declaration form

Here is to be informed that,

The name of employee in the pf declaration is different from the name on employee's pan card. This is impacting me on the kyc updation via pan in epfo.gov.in.
So, we request you to change the employee name in the pf declaration form
