The data collected from the accoplying person is not correctly brought in whereas i am not suffering with hypertionsion but it is mentioned as if a patient of 5 years of hypertension history, it reuirires correction for my insurance ans otherewise,

My wife was admitted for umbilical hernia and she went through surgery. It was in 2011. I don't remember the date. However I can provide her ID as AC01 [protected].
Please advise how I can get her discharge summary.
