Apollo Hospital — Incorrect information in Discharge summary

Case1: Being the son of the patient i was asked to write consent in handwritten with signature and also my mother's, "Stating if insurance is not processed i shall pay in cash" -This is already a term in terms and conditions but still apollo forced us to write it.
case2: Didn't agree to estimated cost of the operation and increased by significant value and caused burden.
case3: Incorrect weight in discharge summary is so much concerning, henceforth every report is incorrect.
case4: As per discharge summary 12/06/2023 is the date but we were released on 15/06/2023.
case5: Upon enquiring with Head superintendent about incorrect weight in discharge summary, it was told it's common to weight errors. Hence not acceptable.

Firstly, I would like to highlight that according to the National Accreditation Board for Hospitals & Healthcare Providers (NABH), it is imperative for healthcare facilities to maintain accurate and comprehensive medical records, including joining documents, discharge summaries. These summaries serve as crucial documents for the patient's post-operative care and follow-up, as well as for any future medical assessments. As a patient, I have a right to receive accurate and reliable information about my medical condition and treatment, and it is essential for the hospital to uphold these standards.
Case-1: My father - Sondi Raju, was admitted to your hospital, and my mother and I were asked to provide an in-hand statement, both individually, to affirm that we would personally pay the full medical bill in cash if our insurance claim is repudiated. I must convey that this request is not only disgusting but also deeply insulting, as it questions our integrity and places an unfair burden on us during an already challenging time it implies a lack of trust and questions my integrity as a patient.
As concerned family members and responsible individuals, we have always upheld our financial obligations with honesty and integrity. The request for an in-hand statement implies that we may resort to dishonest practices or fail to fulfil our commitments, which is both offensive and baseless. Such a request casts a shadow of doubt over our intentions and undermines the trust and respect that should exist between healthcare providers and patients.
Case-2: I had a detailed discussion with the billing team regarding the approximate cost of the medical procedures and services that would be involved in my treatment. Based on their estimation, I was given an approximate value for the total bill, which I found reasonable and agreed to proceed accordingly. However, upon receiving the final bill, I was shocked to discover that the actual amount exceeds the approximate value provided to me by a significant margin this unexpected increase in the medical bill has caused me great distress, as it not only places an undue financial burden on me but also raises questions about the transparency and accuracy of the hospital's billing practices. It is essential for me to understand the reasons behind this sudden surge in cost to ensure that I am being billed fairly and appropriately.
Case-3: Unfortunately, I have discovered several instances of incorrect information in the document, and I believe it is crucial to address this issue promptly to ensure accuracy and compliance with the NABH laws and Abide Consumer Protection rights. The summary inaccurately describes the surgical procedure that I underwent. It fails to mention certain essential details and misrepresents the nature and extent of the surgery. This can create confusion and hinder accurate medical assessments in the future.
I noticed several instances that needs to be rectified urgently. These errors include:
1. Strong Concerns Regarding In-Hand Statement for Cash Payment Assurance in due of my insurance provider.
2. Incorrect information in discharge summary weight and tests. This discrepancy is concerning as it may impact my ongoing treatment plan and potentially lead to confusion or adverse effects and can create confusion and hinder accurate medical assessments in the future.
3. Joined as OPD patient but marked as Emergency, that’s professional misconduct corrects.
4. Initiate a thorough investigation into the discrepancy between the approximate value provided by the billing team and the actual amount charged in my medical bill.
As a patient, I place immense trust in the healthcare professionals at Apollo Hospitals, and I believe it is vital for the hospital to take immediate action to rectify these inaccuracies.
I kindly request the following steps be taken:
1. Review and revision: I urge you to review my discharge summary thoroughly and make the necessary corrections to accurately reflect my medical condition, prescribed medications, surgical procedure details, and post-operative care instructions.
2. NABH compliance and Consumer rights: Regarding accurate and comprehensive medical records.
3. Documentation practices: I also request that you take this opportunity to evaluate your current documents and implement measures to prevent such errors in the future and enhance the reliability and accuracy of medical record.
4. Explanation and Breakdown: Provide me with a detailed explanation for the increase in the bill from1.60 Lakhs dated 5pm 9/6/2023 confirmed by billing team, including a comprehensive breakdown of the charges incurred during my treatment.
5. Patient Rights and Dignity: Reaffirm the importance of preserving the dignity and rights of patients and their families. This includes avoiding any request or action that may be perceived as insulting or demeaning, especially during vulnerable periods such as hospitalizations.
I trust that you will address this matter with the urgency and attention it deserves, ensuring that the necessary corrective actions are taken promptly. I am hopeful that you will provide me with a revised discharge summary that accurately reflects my medical condition and treatment. I urge you to address this matter with the utmost urgency and take all necessary steps to expedite the discharge. Every effort should be made to minimize the impact on the patient and their family, while ensuring the hospital's commitment to efficient and patient-centred care.
