Bank Of Baroda — I don't receive my atm card

Dear sir,
I dont recieved my debit card. I issued my card on 8th august 2017 but i dont receive my card so kindly please send me my debit card on my registered address or reodar branch. I filled form for two times i branch but staff is not working seriously. I filled form before 2 months ago. But staff is not working this work.

Tried to withdraw 5000 on 27 sep from SBI card at your ATM bob 1RDNKOL32 (Ashok nagar tollygunge) balance deducted but money didn't find. Please refund as soon as possible.
