Bharat Petroleum [BPCL] — lost gas book

I have lost my Bharat gas book and i forgot my customer id too. My registered mobile number is my husband number, but we are not living together. Kindly provide me customer id and tell me how to get another gas book. Could you please show me a way to overcome this problem. My details is as follows :-
