Sir, My name is Dommeti Prashanth and I was born in Mumbai on 03/08/1995. But in my date of birth certificate my name is just "Prashant". My surname and letter "h" are missing. Now my permanent address is Hyderabad and I am studying in Kerala. Can you tell me the procedure how to change it. Will it be any problem during Passport application?
