Till date I have Not Received my EPF amount

I (Abdul Haneef Khan) was selected as an Assistant Teacher in Children College Azamgarh (U.P) School Code U.P 12267 and worked there up to 08.02.2001 from 01.06.1999. In the mean time I have received one letter from School office regarding my sanction of final payment of E.P.F but till date i have not received a single penny from your office or information.

Providend Fund not received
Dear Sir / Madam,
I joined Children College, Azamgarh (U.P) on 09/07/1996 and left on 30/08/2004. Hence served the company for more than 8 years. All P. F releasing formalities were done. But I have not received my P.F amount till today even after about 4 years since I left the company. Kindly look into the matter.My P.F account details are as follows:
