Christ University — Cancellation of admission and fee refund

I was selected for LLM and had paid the 140000 fees for the same, however I had to cancel my admission and, I had done it by applying on the application status website by clicking on the cancel admission option.
However, I had received the reply to fill the form in ' Annexure 1'. I am not able to find it on website of christ university and neither I am able to login in the 'application status' portal of the website.

Kindly provide me the relevant requirements for the same since the timeline to gain full refund of only before 15 days of commencement of the classes

I am aware that 5000 will be deducted from the 140000 amount deposited and I will get 135000 only.
