Christian Medical College — non receipt of medical reports in respect of patient hospital no: 707846h name: nanda deb laskar

Dear sir,
I am a patient under the cmc vellore hospital no: 707846h, duly registered as out patient and consulted the physician on 26th aug and 27th aug 2019 and carried out all necessary tests as given by the respective physicians and paid all in advance. After visiting the physicians 1) medicine and another 2) obst & gyne, i have made all payments including the payment for reports from the respective department of medicine and department of obst & gyne @ rs 150/- for each.
But unfortunately, reports were not given even on my request at the offices under the plea of their preoccupation with other works. Hence i have to come back without reports as my flight is scheduled on 8th sept 2019.

I request your good office to take up the matter and pray for my reports urgently for my future need which may be send to me at my email address.
