Cognizant — I'm unable to fill first and last name in cdf-1 form which is sent to my mail by talent aquization cognizant team

I'm Edagotti Sowmya raising this complaint the Cognizant CDF-1 form. I have received a mail to fill the CDF-1 form but while filling the form I'm unable to fill my first and last name as it is not editable and I'm unable to submit the form.

I hope this issue will be resolved as soon as possible.
