Cognizant — Unable to submit CDF-1 form and edit last name

Candidate ID : 24363997

My full name is Aparajita and I don't possess a last name. The link for the CDF-1 form was autopopulated with my details and last name set as blank. It's non-editable and hence I am unable to submit the form.
I need a solution urgently and I kindly request to help me on the same.
