I have not receive admit card for 01 june 2009 MBBS entrance exam My detail are following

Father name- RAJENDRAN M
Mother Name- CHITRA.D
Prospectus number:284120
I DON'T HAVE My application Form no. HOW DO I GET IT ?

How can i get my roll no., test centre add., etc.for MBBS 2009 examination.My app. no. is 288151.
