[Resolved]  govt.org — Birth Certificate

Dear Sir/Madam,

My date of birth is[protected] and I hails from rural area under dist. Etawah o[censored]ttar Pradesh.

Pls Clarify, If I am already listed in Parivar Register of Gram Vikas Adhikari (in Gram Panchayat), then who is the competent authority at Dist. Level to issue copy of Birth Certificate which is acceptable to Passport Issuing Authorities.

Pls someone guide the procedure and any prescribed format of certificate is required.

I shall be very thankful for the same as its necessarily required as a proof of date of Birth who have born after Jan 1989.

In lack of the birth certificate I am uable to apply for Passport, Driving Licence & Correction in my Voter ID.
