Dear Sirs,
This is to inform you that I am left with last cheque bearing number 983839 dated 11.12.2009 to wards annuity payment to me ( A.K.Chatterjee ) the undersigned against Annuity No. 011M[protected] and Policy No. 46004. Kindly take due note of the matter and ensure to send Annuity Payment Cheques for January 2010 onwards as per your rules and practice. The cheques may be sent to my permanent address remains the same as before and is mentioned below for your ready reference.
I may add that my Saving Account No. is [protected] in Punjab National Bank, Shalimar Bagh, Delhi-110088, which the same as before and to which all my previous cheques have been credited.
I am sending Photostat Copy the last cheque with me bearing number 983839 dated 11.12.2009 duly signed by me in token of the fact that I am alive.
