i am suresh kumar agarwal my policy no[protected] exp date 01/11/2011.i am the owner of suzuki GS150R.when i had went to my suzuki showroom to repair my vechile they told me that the part which is damaged will be availabe after 1 or 2 weeks.so, then i had went to my insurance company to fill the form at that time they had not given me the form and my policy is also going to expire on 1st of nov.so plz help in giving the form.my phone no. is [protected].plz help in solving the problem
