It is respectfully submitted that I made an online payment of ₹15, 482/- on 13/10/2025 through the PhonePe app while generating a car insurance policy via Hero Insurance Broking Company. The payment was successfully transferred to the ICICI Bank account of The Oriental Insurance Company Ltd., but the policy was not generated, and my payment remains stuck.
The details of the transaction are as follows:

• Payment Date: 13/10/2025
• Mode of Payment: PhonePe App (UPI)

• Beneficiary: The Oriental Insurance Company Ltd. (ICICI Bank Account)
I therefore request you to kindly refund the amount of ₹15, 482/- to my bank account mentioned below at the earliest, as the policy was not issued against this transaction.
