army — Lost PAN Card

I lost my PAN card and i want to apply for my new PAN card. I have filed a complaint with the local police station and have an affidavit. My PAN no is AXHPS3774M. I want the details of that card so that i can apply for a new card.

how can i get my lost pancard back. what r the procedures to get it back.

1. My existing PAN cared is AZUPS6500J.
2. I'm serving army officer.
3. Photograph on PAN card has faded to extent of being non recognizable.
4. Request reissue fresh card.

I have lost my PAN Card . My PAN NO. ADGPP2692C. I have already gave application for issue duplicate PAN Card. Since long i have wait for that but there is no reply from your side. I request you to issue the same.
