Pan card — Renew my pan card

i am paramjeet singh saluja my pan no. is ( BHRPS5863C) my pan card is distroy so i want to get is replaced by new pan card.

I am using my Old Black & White PAN Card No: AAXPB5586F. It is mutilated and lamination is getting apart. I want to get it replaced by new coloured PAN CARD
