Kindly note that my wife Bena Maria Mascarenhas G.J. (Pan AAGPF 5085J ) and I, Gloria Furtado (Pan AAGPF5084K) have filed for IT return for the assessment year 2007-08. But till now we did not recieved the refund value.
Again we filed for IT return for the assessment year 2008-09. The same story. Not recieved refund value till for any assessment year.

I have to file for the same again for this year 2009-10.


Please solve the matter without any further excuses but only prompt solutions will be of great help to resolve my financial crunch to meet my fimily survival.

I was given a credit card by SBI and a Health card was also issued, The premium for the health card was debited from my credit card and i was supposed was to repay the same.
The Credit card and the health card were delivered to my address and till the time i have not given my acceptance for the credit card and healt card everything was fine and people from SBI DSA and customer care used to call me for all the help in this word.
But every thing was changed the day i received the credit card and health card.
The credit card and health card both has wrong spelling of my Name.
My Name is Sachin Bansal, While the credit card and the health card carried the name as Sachin Bansel.
I requested many a times to the customer care for getting the spelling corrected so that i can use the cards. Afetr lot many requests the credit cards were corrected but no body paid attention to the changing of the name on Health card.
The Premium to be paid was some where around rs 3000 in Apr 2007, And because of the non payment the amount has increased to Rs 13000.
I did'nt make the payment as the heath card with wrong spelling is of no use to me.
Now i am getting calls from SBI recovery / Legal Dept. The person uses very bad language. Has told me that he will make sure that my life is ruined, I will be declared a defaulter, No bank will give loan to me, I will be denied Visa.
I have one question here how am i liable for the mistake of SBI.
Am i supposed to pay the penalty for the wrong done to me just because SBI is big and have many a lawyers to defend.
I will request the concerned to look in to the matter and advise how to get out of this mess.

i apply for my pan card on first of july 09 on line CORRECTION IN DATE OF BIRTH WHICH IS WRONG . that was submitted to the department of income tax in the month of may 1999.whiches is not get yet . so i apply for same and attached all relevant documents as per your desired documents list i.e.

1. Proof of identity - WATER BILL OF MY HOUSE
2. Proof of address - COPY OF MY RATION CARD
3. Proof of Pan - COPY OF PAN ALLOTMENT LETTER
4. date of birth - COPY OF MATRICULATION MARK SHEET

with all these documents and print of on line form i send to you by post on 2nd of july 09. I deposit rs.94 in your account through HDFC BANK.after that i get a mail form your side that documents are not valid tan i mail the documents again. after that i faxed my documents july 15 09 . after that i sent coy of on line application form following documents again on july 21 st 09 by speed post to you.
