[Resolved]  Provident Fund Commissionerate, West Zone / Eps Cerificate — Not received the EPS Certificate

Dear Sir,

With respect to above I have neither received the EPS Scheme Certificate nor any communication since then.
Now I request you to arrange the same at the earliest or if permits arrange the entire amount in my credit including accumulated interest as on date under Claiming withdrawal benefit provision since I am not in any employment for the last two years due to health grounds.
Would request you to arrange the withdrawal arrangement keep above in view.
I am herewith enclosing the copy of said reference (2) along with Form No.10-C (E.P.S)
