Regional Passport Office — Wrong entries in reissue of Passport

My Husband's Passport was expiring in march 2007 so we applied for the reissue of passport by filling the concern form and submitted it in feb 2007. The new passport came after three months with lot's of mistakes.

1) name : actual name is Bollapragada Ganesh
Name in new passport Bolla pragada Ganesh
2) Spouse name : Actual Name Bollapragada Venkata Mahalakshmi
Name in Passport Bolla pragada Venkata Mahalakshmi
3) Birth Place: Actual : Kakinada
Place in New Pasport : East Godavari

4) Place of Issue: Actual : Chennai
In New Passport : 'hennai
we went and asked for the procedure for corrections in the passport as he has to apply for h1 visa but nobody responded and some said you have to submit form 2 with the corrections to be made and some said you have reapply for new passport by form1 and some are saying you have get a letter from the first class megistrate that the place of birth is enered wrong make the required changes in the passport

but these mistakes were done by the officers .

we are in a confused state that how rectify these mistakes and a new passport so that he can apply for h1 visa .
