I have a family medicare policy in the United India Insurance Company Ltd. My policy no. is 160700/48/11/06/00000986. The period of insurance is from 27/06/2011 to 26/06.2012. The name of the insured is Mr. Dharmendra Ahuja. I have not received my policy identification card for the individual which is now linked to paramount health services pvt. ltd. having a PHS ID. While checking my current policy information it was brought to my notice that my address has been wrongly written even after sending a form for correcting the address earlier.
