With reference to the letter HO/ACC/FP/S-I/679211 dated 19.10.2007 from Usha International Ltd., New Delhi in form no. 10-B ( F P F) for the refund of family pension contribution of Rajnish Kumar Sharma Holding A/c No. DL 2481/E/9187 date of commencement of membership 01.03.2004 and date of birth 05.05.1975 and the letter was enclosed with advance receipt and Form no. 7 ( F P F ).

I would like to bring to your attention to the fact that I have not received the amount as of now and you are therefore requested to look into the matter and arrange to refund my family pension balance.
