Dear Sir, I had applied an application for change of name in my pan card one month ago.At present my FATHER's name has been written inplace of my name.I had submitted this complaint to UTI OFFICE,ZONAL MARKET SECTOR-10 ,BHILAI,DISTT.-DURG (C.G.) .But they are also unable to process my application for further corrections.DETAILS OF MY PRESENT PAN CARD. I am a government servant having pan no AGXPM5276B. I had given pan no a long ago, but due to mismatching of my fathers name i had not received pan card yet. Repeated request on this was not fruitful.

Address 615/25, sharadha, 5th cross, I main
Raghavendra colony, Billekakhally,
Bangalore-560076.
Since in karnataka first name, sir name is not much familiar, we use initials. Please rectify the details and issue pancard as early as possible.
